feat: add session-start prompt templates for all surfaces

Four variants: Standard (Projects + Graphiti), Quick, Claude Code CLI, and Desktop/browser (no MCP). Covers the full surface matrix. Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

+- Graphiti is only queryable where the MCP connector is live (claude.ai web, Claude Code CLI).
