D&D3.5 Spacing fixes, color/error fixes etc
...
Reverted old pull due to an issue/mistake; This commit has fixes to
solve the spacing/alignment issue previously seen in Chrome, reverts the
colorizing of the skill rank box at zero ranks for trained only skills
(and thus solves the error it produced), fixes the appraise macro issue,
etc.

D&D3.5 update, cosmetic +
...
added notes areas for saves and skills to use in the roll template notes
property. Added support for epic levels by adding Epic Save Bonus and
Epic Attack Bonus. Switched some value= to placeholder= to make text
fields a bit more friendly to edit. Adjusted the size of several fields
and removed some redundant styles.
Also added additional checkbox to CSS to support the save notes.

Cosmetic fix to Spell section label, rename strmodrab to dexmodrab to
correctly follow naming convention, correctly use armorclassshieldbonus

Updated .css file for roll templates, adjusted character sheet names to
put in roll templates. Added comment to .json file. Depreciated file
without roll templates in directory (with additional file with tabs) but
not linked via .json file; available for mentor users if desired.
