Ringworld sheets v4.5 — bleed fix, 20 white circles, white boxes

- tbl_y pushed 20pt below header bar in all three sections
  (regular hloc, puppeteer hloc) so no text ascenders reach the bar
- Combat reference sub-headers also shifted down 4pt extra
- HP tally: squares replaced with 20 small white circles (r=5pt)
  with tiny numbers 1-20 below each; white fill so crossable with pen
- CURR HP and ARMOUR write-in boxes: dark → white fill with
  accent border so they can be marked/crossed in black pen
