Added Initative + Minor Fixes / Tweaks

Added a seperate "roll for initative" button that automatically adds
tokens to the turn tracker.

Also:
* Added the epic dice token limit to the sheet itself.
* Renamed character name field so it hooks up correctly with the one in
the bio correctly.
